In principle, you have two options for designing visit reports. Either directly through appropriate configuration by the blue-zone support or the independent design in forms.office.com. These independently generated visit reports are called dynamic visit reports. In this How-To chapter, we will show you how to integrate a customised visit report into POLUMANA® using forms.office.com. We also give you tips on how to use special functions in forms.office.com for polumana.

INTEGRATION OF DYNAMIC VISIT REPORTS IN POLUMANA®
- Click on Share within forms.office.com and then on Copy

2. Switch to the POLUMANA® app and open Settings/Registration&Databases and click on Create form template from MS Forms

3. Now paste the copied content into the line and then click on the link button

The visit report you have created will now open. Here you can test the visit report and check whether the keywords are working as required.
4. Now close this dialogue and save the xml file on your device.
5. Now open the Azure Storage Explorer and open the FileShares / app-template folder in your subscription.

6. Open the ‘template-content’ folder
7. Now download the previously saved xml file into this directory

8. After the newly created xml file has been uploaded, change to the ‘template-configuration’ directory
9. Download the Excel AppTemplate.xlsx (double-click) and open this file.

10. Enter the following data:
- ID – any name of your visit report template
- Content File Name – the name of the saved xml file
- Type – FORM_VISIT_REPORT_V3

11. Save the AppTemplate.xls and upload it again in the Azure Storage Explorer. (Upload)
Your template will now be processed. This process can take up to 30 minutes
12. Open the POLUMANA® app and download a Delta update. The new visit report is now available for you.
SPECIALITIES IN THE DESIGN OF VISIT REPORTS IN FORMS.OFFICE.COM
Keywords must be specified POLUMANA® keywords in the subtitle or in the description of a question.
For example:

You can use the following POLUMANA® keywords when designing the visit report:
- PartnerIdSelectionProvider: Used to select customers in the visit report
- ContactIdSelectionValueProvider: Used to select contacts
- NewPage: Create new site
- Time: Defines a time field
- IsCheck: Control of jump labels for checkboxes
You can use the POLUMANA® keyword PartnerIdSelectionProvider to select customers in the visit report.

You can use the polumana keyword ContactIdSelectionValueProvider to select contacts in the visit report. Proceed in the same way as for selecting customers.
You can use the POLUMANA® keyword ContactIdSelectionValueProvider to select contacts in the visit report
The creation of a new page is controlled by the polumana keyword NewPage via sections. First create a new section for the question to be placed on a new page.

Then enter the keyword ‘NewPage’ in the Description field and formulate your questions within the new section.

Entering a time can be controlled with the POLUMANA® keyword Time.

Whenever you want to integrate certain branches based on a selection with a single select (radio buttons), you must use the keyword ‘IsCheck’. To do this, write ‘IsCheck’ in the subtitle of the question and click on ‘Add branch’.
